Eligibility for Supported Living
To qualify for Supported Living services, an individual generally must:
Have a qualifying I/DD diagnosis
Be Medicaid eligible
Be approved for:
Innovations Waiver services or
Demonstrate the ability to live safely with 1:1 support
Have an approved service authorization through their LME/MCO
Shine works with care coordinators and families to determine if Supported Living is appropriate
